Sliding Fee Discounts
We are here for you! Our discount program is available to all eligible patients without regard to age, gender, race, sexual orientation, creed, religion, disability, or national origin.
Eligibility for Minnesota Community Care’s discount program is based on income and family size using the Federal Poverty Guidelines (FPG). Patients must reapply for the discount program every six (6) months, or whenever there is a change in family size or income. This program is designed to make your care affordable for you. However, you may still be responsible for some of your medical charges.
Please note that as of August 19, 2024, patients who have third-party insurance will not qualify for the discount (except for the Ryan White program). If you would like to set up a payment plan, please apply through the billing department.
All patients may still qualify for discount programs at the pharmacy, even if they have insurance.
To Apply for the MCC Sliding Fee Discount Program
Please see the application below for more information. You may either print and complete the application at home or receive the application at the front desk at the clinic. In either case, please bring your proof of income documents to the clinic with you each time you update your application. If you do not provide proof of income within 30 days of your application, you will be responsible for the full cost of your care.

If you are approved for the discount program*, MCC will collect a nominal fee up front for most visits. The nominal fee is $40 for medical visits and $50 for dental visits. After the visit, the patient may receive a bill for a percentage of the charges from that visit. The nominal fee will be applied toward the incurred charges. Not all appointment types have charges associated with them.
*Some services are not covered by the discount program.
INS physicals are NOT covered by any MCC discount program.
Some dental procedures may also not be covered by the discount program, and these services will require payment up front before the procedure can be performed.
