Affording Your Care
Minnesota Community Care is the largest Federally Qualified Health Center in the state of Minnesota. We believe in providing health care for all, regardless of income or access to insurance.
If you cannot afford your care, we will work with you to determine a payment plan, help you access insurance, or see if you qualify for discounted services through our sliding fee scale.
If you have health insurance, we accept:
- Medicare
- Medicaid
- Many private health insurance plans
We recommend that you review your health insurance plan information or call your insurance company before your visit to make sure your fees will be covered.
Please note that as of August 19, 2024, patients who have third-party insurance will not qualify for the discount (except for the Ryan White program). If you would like to set up a payment plan, please apply through the billing department.
All patients may still qualify for discount programs at the MCC pharmacy, even if they have insurance
No patient will be denied service,
due to an inability to afford their care .

Many of our patients qualify for health insurance. MNsure is Minnesota’s health insurance marketplace where you can shop, compare and choose health insurance coverage that meets your needs.

If you are interested in our sliding fee discount program:
We offer a Sliding Fee Discount Program for patients without insurance. Eligibility is determined by your financial situation and family size.
To qualify, you must do the following:
- Complete the application
- Discount Program Application (English)
- Discount Program Application (Spanish)
- Provide information for all family members currently living with you
- Provide proof of income
(Options include: tax form, last pay stub from your employer within the last 3 months, bank statements, or a written letter from your employer).
To remain in the program, you must reapply every six months or whenever there is a change in your current family size or income.
Insured patients may be eligible for the Sliding Fee Discount Program to reduce their copayments and deductibles.
